Arthritis Pain

Arthritis is a common condition affecting old people. It also affects more than 10% of the adult population.

There are more 100 different types of arthritis which exists.

Since each type of arthritis varies from each other, thus even the approach to treatment for each type of arthritis is different. Therefore accurate diagnosis of the type of arthritis the individual is suffering becomes utmost important.

Correlation Between Arthritis And TMJD (Temporomandibular Joint Disorder)

There is a strong association found between arthritis and TMJD.

TMJD is an Infratemporal fossa catastrophe, occurring due to inward disc (medial) displacement of the Jaw Joint or TMJ.

Malocclusion of teeth or mal-alignment of jaws or TRAUMA to the jaw joint causes the joint disc (articular disc) to get displaced medially.

This happens due to abnormal teeth grinding, which causes a tilt in the jaw joint leading to abnormal residual forces in the jaw joint.

Inside or on the inside of joint exists major structures travelling from and to the brain like nerves and blood vessels. This leads to compression of these structures.

The first nerve to be encountered, at a distance of just 0.5 cm or an ants length is the accessory nerve or the number 11 cranial nerve. The job of this nerve is to maintain the head on shoulders at the “COG” or the center of gravity.

Irritation of this nerve alters the muscle function and hence the head position, thus ultimately putting various body joints under undue pressure thereby resulting in arthritis.

Common Types Of Arthritis

Types Of Arthritis —

  • Osteoarthritis- also referred as “wear and tear” arthritis. It is a degenerative type of arthritis.
  • Rheumatoid Arthritis- It is an inflammatory type of arthritis.
  • Gout- occurs as a result of accumulation of crystals in the joints.
  • Other Types Of Arthritis- Psoriatic arthritis, Lupus arthritis and Septic arthritis.

Osteoarthritis is also referred as ‘WEAR AND TEAR ARTHRITIS’ or ‘DEGENERATIVE ARTHRITIS’.

It is the most common type of arthritis affecting the general population. Seen most commonly as an aging phenomenon.

It is a chronic joint condition resulting from over usage of joints.

It may also occur as a result of obesity that may cause increased load on weight bearing joints or sports that may cause damage to the joint.

Osteoarthritis is more common in middle-aged women.

Osteoarthritis is commonly seen affecting the weight bearing joints — like knees, hips, feet, and spine.

It is a chronic condition which gradually develops over years.

There is no major symptom experienced by the patient except for pain in the affected joint. Usually there are no other signs of fatigue or tiredness as associated with other types of arthritis.

In osteoarthritis, the cartilage undergoes gradual breakdown.

Cartilage is a slimy material covering the end of the bones and functions as the body’s shock absorber. As a result of damage (as a result of over usage) or injury (as in case of demanding sports) or extra load on knees (as in case of obesity), this cartilage tends to wear off and doesn’t work as efficiently as should be working normally. With wearing away of the cartilage, the cushioning effect of the joint is compromised thereby resulting in pain and sometimes it may even result in grating sound due to the rubbing of the roughened cartilage against the joint bones.

Breakdown of the joint cartilage may result in inflammation in the joint lining.

Symptoms Of Osteoarthritis

  • Deep, aching joint pain
  • Difficulty while sitting or bending over
  • Joint stiffness on waking up from sleep
  • Pain on walking
  • Joint felt warm on touch
  • Difficulty in gripping objects
  • Stiffness in the joint after resting for some time
  • Joint swelling
  • Limitation of joint movement

Rheumatoid Arthritis

Rheumatoid arthritis is a kind of inflammatory arthritis.

Women are more commonly affected by osteoarthritis (3:1).

About 1- 3% of osteoarthritis women eventually develop rheumatoid arthritis in their lifetime.

Rheumatoid arthritis usually has an autoimmune etiology. That means one’s own immune system starts attacking the normal body cells.

This autoimmune malfunctioning is usually seen affecting the body joints.

With progression, this chronic inflammatory condition of the joint may cause severe joint deformities and damage.

Almost one in every five patients of rheumatoid arthritis tends to develop lumps on their skin referred as RHEUMATOID NODULES. These nodules commonly develop over joint areas that are normally subjected to pressure like knuckles, heels or elbows.

Patient may experience the symptoms of rheumatoid arthritis either can gradually or sometimes suddenly.

Symptoms Of Rheumatoid Arthritis

Compared to osteoarthritis, symptoms of rheumatoid arthritis are generally more severe.

It includes-

  • Joint pain-sometimes pain experienced in one body joint and sometimes felt in several joints like hand, knees and feet.
  • Fatigue
  • Appetite loss
  • Joint stiffness
  • Swelling in your hands, wrists, elbows, shoulders, knees, ankles, feet, jaw, and neck.
  • Symmetrical pattern of involvement of joint.
  • Swelling of the joint interfering with the normal activities.
  • Joint may feel warm on touch.

Arthritis Pain Treatment

Pain relievers-

Acetaminophen (for relieving mild and moderate arthritis pain)

Non-Steroidal Anti-Inflammatory Drugs (NSAIDs)- such as Ibuprofen, Diclofenac or Naproxen Sodium.

TOPICAL CREAMS-may also help to alleviate arthritis pain temporarily.

Supplements- like Glucosamine and Chondroitin sulfate. Combination of glucosamine and chondroitin sulfate have been found effective in providing pain relief, though there is no definite proof in medical studies that they are helpful in every arthritic patient.

Injecting potent anti-inflammatory agents (like Synvisc One) into the joint helps in reducing pain and inflammation. Ozone Gas injections have also been found extremely effective in pain relief.

TMJD Treatment

To get long term relief from arthritic pain, patient needs to treat the underlying cause which in majority of cases found to be the TMJD.

TMJD treatment requires correction of TMJ residual pressures through the dental occlusion correction. TMJD correction is more of a Bio-Engineering problem rather than a Biological problem.

It becomes utmost important to confirm it with the aid of various sophisticated and most advanced diagnostic technology(AVAILABLE ONLY AT Zental Tmj & Body Pain Division  BODY PAIN TREATMENT CENTRE, only SUPERSPECIALIZED DENTAL clinic in INDIA and PERHAPS IN ASIA TOO), like-

Once the diagnosis is confirmed and connection between TMJD and arthritic pain has been established, various treatment procedures can be done to eliminate back aches from the roots. It includes-

  • Home care remedies
  • Medicines-pain relievers, muscle relaxants, anti-depressants.
  • Orthotic/splint
  • Orthodontic treatment
  • Veneers, Crown and bridges
  • Botox

Stem Cell/Platelet Rich Plasma (PRP) therapy

This therapy involves injecting platelets drawn from the patient’s own blood for rebuilding a damaged tendon or cartilage. It helps not only relieving the pain, but also helps in fastening the healing process. After drawing the blood from patient’s body, it is centrifuged for 15 minutes to separate the platelets out from the blood. The separated out platelet-rich plasma is then injected into the damaged portion of cartilage or tendon for rebuilding it.

Surgery is considered as the last treatment resort when all other treatment modalities have failed. Success rate of such surgeries is limited to 60-70%.
